We, unfortunately, have to move to on-prem. Does anyone know if I can just change the address the agent talks to on each machine or is there more to it than that?

There are a few ways but the easiest way to retain your clients is to first set up the onpremise server correctly and then place an Access client installer into the cloud instance's toolbox. You can then Run the exe as a Tool from the Host page to move the clients to the new server without risking loss of connectivity.
